We received your child's enrollment information for health insurance coverage through the New York State of Health Marketplace. There are a few remaining steps needed to complete your child's enrollment:

• You will soon receive an invoice via postal mail. It will tell you the premium payment amount that is due and the date your payment is due to MVP. In order for MVP to complete the enrollment process and begin your child's coverage, the first month's complete premium payment must be received by the due date noted on the invoice. Your child's coverage will not start until we receive the full premium amount for the first month of coverage.
• Please be sure to include your child's transaction ID number on your check and return the invoice with your payment. Your child's transaction ID number can be found on your premium invoice.
• Once MVP receives your payment, your child's enrollment process will be complete. At that point, you can expect to receive your child's Member ID cards and an official welcome packet via postal mail.
